How to Print a Request Form

Have you read about this program?  If not, please do so before requesting a quilt.

To request a quilt you need to print out and complete a request form and mail it to us.  Unfortunately, due to abuse problems with our online order system we have had to revert to a manual process.  We apologize for this inconvenience but encourage you to print the request form and mail it to us (sorry, but we can no longer accept phone, FAX or e-mail requests).

Madison, diagnosed before age 1

Sweet little Madison was diagnosed with type 1 diabetes the day before her 1st birthday.  Madison was the very first child ever to receive a quilt through our program.

Alyssa, diagnosed at age 2 loves her quilt~

Alyssa, age 3, from California was diagnosed with type 1 diabetes at the age of 2.  She loves her free quilt from us as well as her IPump teddy bear!
