Step 5.  Mail your application to us

We will not begin processing your application until we have all information.  It is important that you attach prescriptions, financial documents, and proof of identity.

The most common reasons we have to return applications are because someone fails to:

• Provide a photocopy of ID
• Include any financial documentation
• Does not sign the application and attachments

When you are done with your application mail it with any attachments to:

We do not accept applications via email, phone, or FAX.  You can call or email to check the status of an application but please know we cannot tell you if you qualify unless we have an application on file.
