   Sponsored Assistance Applications

What is a Sponsored Application?

We offer two ways to apply for assistance:  with an Individual Application or with a Sponsored Application.

A Sponsored Application is an application that a client can submit with the help of a patient advocate professional.  Advocates include a social worker or case worker, or licensed health professional that treats the applicant with diabetes.  Sometimes, a patient advocate can be a member of the clergy provided the applicant has a long-standing relationship with the clergy member or is currently receiving financial assistance from a particular church or religious organization.

A patient advocate, which we call "Sponsors" must be able to:

  • Verify as a professional (not as a friend or relative) who the applicant is because they have already met the person face-to-face and has seen proof of identity; and
     
  • Has seen documentation as the the applicant's financial and insurance situation and is willing to sign a statement that the Sponsor believes the applicant has hardships that make it difficult or impossible for the applicant to afford their diabetes care on their own.

Also:

  • A sponsor may not be a friend or a relative even if they are also a  professional patient advocate.
     
  • A sponsor must either be a volunteer or employee of an organization that either provides assistance or helps people in need find assistance.
     
  • A sponsor may not evaluate your situation simply from a phone interview or if they have not seen documents to support your need.

Because your sponsor may be required to provide personal, medical, and/or financial information about you and your situation you need to sign a release form that allows IPump to communicate with your Sponsor in order to assist you.  This form is contained in the Sponsored Application.

Benefits of submitting a Sponsored Application

A sponsored application is a shorter application for assistance because it does not require proof of financial data (although we reserve the right to request this information if the Sponsored Application does not seem to be sufficient or accurate information).

Because another assistance professional has already assessed and verified your situation your application may be easier to process and therefore approved faster.
