   Individual Assistance Applications

What is an Individual Application?

We offer two ways to apply for assistance:  with an Individual Application or with a Sponsored Application.  If you have a social worker or professional patient advocate helping you find and apply for assistance you may wish to see if you can submit our Sponsored Application instead.

An Individual Application is one that can be submitted by anyone in need who has diabetes and is residing legally in the United States.

With an Individual Application you will need to show some sort of evidence of who you are (i.e., a photocopy of a driver's license) as well as some sort of documentation proving your need for assistance.

Proof of hardship may include:

  • Current bank statement and paycheck stub;
  • Copies of letters showing you are already receiving assistance through another hardship or supplemental program;
  • Copies of delinquent medical bills;
  • Letters from insurance companies denying claims or coverage;
  • Copies of insurance policy restrictions (that shows diabetes is not covered); or
  • Anything else that shows, despite income, you are having a hard time.
