Eligibility

To qualify for monthly assistance you must: reside legally in the United States; have diabetes; and take insulin, BYETTA, or Symlin for your diabetes, and:

  • Show proof of what you pay out of pocket for injected medications each month (receipts required);
  • Have no insurance coverage for the injected drug; or
  • Have a co-pay of at least half the cost of the medication (i.e., if insulin costs $80.00 and your co-pay is at least $40 then you may be eligible for cash assistance); or
  • Have satisfactorily demonstrated other reasons or causes for financial hardship.

About the program

This program provides emergency, temporary financial assistance to qualifying individuals to help cover the cost of diabetes injectables care for one to three months.  This program is not intended as a long-term solution and we do not send medications.  This program is for those who have trouble affording co-pays for injectables and those who have no insurance coverage.  The maximum assistance anyone can receive from this program is currently $150.00.  It is our hope that this benefit amount will increase in the near future.

The following items may be covered in full or in part:

Injectables Covered Maximum Amount Covered
Insulin (vial) - All types, in vials (we do not cover inhaled insulin, which is being discontinued). Maximum $50/month for all types of diabetes
Prefilled Insulin - In pre-filled cartridges or pens. Maximum $50/month for all types of diabetes
Exenatide (BYETTAâ„¢) - Covered when prescribed specifically for LADA or type 2 diabetes.  At this time we are not able to offer assistance to those who may be using BYETTA to treat PCOS, pre-diabetes, or a medical condition other than diabetes.

Maximum $50/month for the treatment of LADA.

Pramlintide (Symlin®) - For type 1 diabetes only.  To received coverage if you have type 2 diabetes your doctor will need to indicate that you are on insulin and why Symlin is necessary for your medical treatment.

Maximum $50/month for type 1 diabetes.  (In some instances, Symlin may also be covered for certain type 2 diabetic patients.)

Glucagon - This program only applies to those who are currently using insulin.

Up to $50 per glucagon kit (up to 2 kits).

Note 1 - Latent Autoimmune Diabetes in Adults (LADA) is a form of diabetes that leads to the destruction of the beta islets cells in the pancreas.  The patient will eventually become insulin dependent the same as in type 1 diabetes.  BYETTA is sometimes prescribed to LADA patients in early stages to help prevent or slow further destruction of the beta islets cells. Unlike with many type 2 patients, LADA patients cannot control blood sugars or prevent pancreas burnt-out with lifestyle changes and are often limited to few medical options.
